ABSTRACT

Objective To analyze the epidemiological characteristics and risk factors of myocardial injury in diabetes patients with coronary heart disease in Qingdao, and to provide a theoretical basis for the prevention of myocardial injury in diabetes patients with coronary heart disease. Methods A total of 196 diabetes patients with coronary heart disease admitted to the Affiliated Hospital of Qingdao University from June 2019 to June 2020 were selected. The patients were divided into myocardial injury group (n=39) and non-myocardial injury group (n=157) according to whether myocardial injury occurred during hospitalization. Four ml of fasting elbow venous blood was collected from all subjects. The serum cTnT, BNP and CK-MB levels of the two groups were compared. The clinical data of the patients were collected from the medical record system, including gender, age, history of taking lipid-lowering drugs, BMI, diabetes mellitus, course of coronary heart disease and serum TG, FPG, PBG, HbAlc, HDL-C, LDL-C levels, etc. Univariate analysis and logistic regression analysis were used to analyze the influencing factors of myocardial injury in patients with diabetes mellitus complicated with coronary heart disease. Results Among of 196 diabetic patients with coronary heart disease, 39 cases (19.90%) had myocardial injury, including 21 males and 18 females. There was no difference in gender between the two groups (χ2=0.105, P>0.05). The age of patients in the myocardial injury group (64.78±5.67) was significantly higher than that in the control group (59.72±5.12) (t =5.016, P<0.05). The serum levels of cTnT, BNP and CK-MB in the myocardial injury group were significantly higher than those in the control group (P<0.05). There were significant differences in the course of coronary heart disease, serum Hcy, TG, FPG, PBG, HbAlc, TG and LDL-C between the two groups (P<0.05). Increased Hcy (OR=2.673), increased FBG (OR=3.681) and increased LDL-C (OR=2.912) were risk factors for myocardial injury in patients with diabetes mellitus complicated with coronary heart disease (P<0.05). Conclusion The risk of myocardial injury in patients with diabetes combined with coronary heart disease in Qingdao area is high, which is closely related to the increase of postprandial LDL-C, FBG and Hcy in patients. Active intervention should be given to reduce the risk of myocardial injury.

ABSTRACT

ObjectiveTo investigate the clinical effect of laparoscopy, choledochoscopy, and duodenoscopy combined with T-tube-free drainage in the treatment of gallstones with common bile duct stones. MethodsA retrospective analysis was performed for the clinical data of 564 patients with gallstones and common bile duct stones who were admitted to Department of General Surgery, Pudong New Area People’s Hospital, Shanghai University of Medicine & Health Sciences, from December 2017 to December 2019. According to the surgical procedure, the patients were divided into laparoscopic transcystic common bile duct exploration (LTCBDE) group with 191 patients, three endoscopies+laparoscopic common bile duct exploration and primary suture (LBEPS) group with 138 patients, and endoscopic retrograde cholangiopancreatography (ERCP)+endoscopic sphincterotomy (EST)/endoscopic papillary balloon dilation (EPBD)+laparoscopic cholecystectomy (LC) group with 235 patients. The three groups were analyzed in terms of the general data including sex, age, bile duct diameter, and stone size/number, and surgical condition and complications were compared between groups. A one-way analysis of variance was used for comparison of normally distributed continuous data between groups, and the Kruskal-Wallis H test was used for comparison of non-normally distributed continuous data between groups; the chi-square test was used for comparison of categorical data between groups. ResultsThere were significant differences between the LBEPS group, the ERCP+EST/EPBD+LC group and the LTCBDE group in hospital costs, drainage volume, time to first flatus, length of hospital stay and time to extraction of drainage tube (F=416.40, 7.80, 33.99, 143.70, and 13.08, P<0.001, P=0.020, P<0.001, P<0.001, and P<0.001). Compared with the LBEPS group and ERCP+EST/EPBD+LC groups, the LTCBDE group had significantly lower hospital costs and a significantly longer time to first flatus, and significantly shorter length of hospital stay and time to extraction of drainage tube(all P<0.05). No serious complication was observed after surgery, and there was no significant difference in the incidence rate of complications between the three groups (P>0.05). All patients were discharged successfully after surgery. ConclusionThe three minimally invasive surgical procedures combined with T-tube-free drainage achieve the goal of little trauma and pain, fast postoperative recovery, and few serious complications, among which LTCBDE has the lowest treatment costs and the best postoperative recovery.

ABSTRACT

Objective:To analysis the clinical characteristics of The clinical characteristics of using automated external defibrillation in the public place,To explore the feasibility and effectiveness of AED application in public places in China.Methods:From January 2014 to April 5, 2019, 54 cases of on-site emergency medical records of AED use in public places in China were analyzed retrospectively from three aspects: patient and AED user attributes, and AED clinical performance.Results:After field application of AED analysis, 54 patients did not have out of hospital cardiac arrest in 9 patients; cardiac arrest in 45 patients, cerebral resuscitation in 40 patients (88.9%), death in 5 patients (11.1%), one of them died in hospital. The accuracy of AED for defibrillation rhythm recognition and defibrillation recommendations was 100%. The success rate of shock to VF was 97.22%, and that of non pulse VT was 100%. The data shows that AEDs of different brands show clinical effectiveness in the core indicators of work. The operation level of the rescuer determines the critical time of AED shock, which is closely related to the prognosis of the patient ( P<0.05) . Conclusions:AED is reliable and effective in electric shock decision and performance.The overall efficiency of AED application can be improved by strengthening training, shortening the critical time of electric shock, rational configuration and effective management.

ABSTRACT

Objective@#To explore the effect of reconstructing the finger extension function by using partial flexor digitorum profundus tendon transposition.@*Methods@#Twelve patients were treated by using the partial proximal end of the tendon core of flexor digitorum profundus tendon regionⅠand part of region Ⅱ to cut off to the base of the distal digital phalanx, and puncturing at the end of the phalanx with a 2.0 drill. The tendon strips were pierced to the back overlap sutured to the proximal end of the avulsion of the extensor tendon .@*Results@#In 3-18 month follow-up, the average follow-up duration was 10.6 months. According to Dargan function evaluation method, 12 cases were excellent and one case was poor.@*Conclusions@#The method of reconstructing the extensor tendon by partial flexor deep tendon is a reliable method for the treatment of Mallet finger.

ABSTRACT

Gauze packing and drainage is a routine treatment for various types of fistula and sinus pressure ulcers. Because the external orifice of pressure ulcer or fistula is relatively small and the sinus deep, the conventional gauze packing has many drawbacks. Under the situation the operator is not skillful enough and no appropriate tools, it is necessary to pull the external orifice by tweezers to dilate the sinus and perform gauze packing and drainage, which brings great pain to patients, possibly leading to wound enlargement and delayed healing. At present, there is lack of auxiliary tools to improve gauze packing. Therefore, we developed and designed a gauze filler for packing and drainage of various kinds of fistula and sinus pressure ulcers, the device has the following advantages: good drainage effect, simple operation, low cost, safety and effectiveness. It not only improves the comfort of patients, reduces the number of dressing changes, reduce the number of dressing changes, shorten the hospitalization time and pay expense, but also reduces the burden of medical workers thus it is worthwhile to popularize and apply the newly designed gauze filler in clinical practice.

ABSTRACT

Objective@#To observe and study clinical efficacy of restruction of finger pulp using tibial flap.@*Methods@#From January 2009 to October 2013, we reconstructed the 20 finger pulps with free tibial flap of the second toe.@*Results@#20 flaps survived completely and were followed up for 6 months to 2 years, with an average of 1 year. There were no obvious scars in the reconstructive finger pulps. And the reconstructed finger pulps all had good appearance, texture, a fingerprint point discrimination at 6-8 mm, and finger function recovered well.@*Conclusions@#The shape of finger pulp after reconstuction with the free tibial flap of the second toe has the similar appearance to the normal finger pulp and a good sensory function, which is one of the best ways to reconstruct the finger pulp defects.

ABSTRACT

Objective To evaluate the clinical value and effect of biofeedback training therapy for functional constipation in elderly patients.Methods A total of 78 elderly patients with functional constipation were treated at the People's Hospital of Pudong New Area,and were divided randomly into two groups.The control group (n =39) received routine treatment plus oral polyethylene glycol,while the experimental group (n=39) received biofeedback training treatment in addition to what was offered to the control group.After treatment,constipation symptom scores,anorectal dynamic parameters and clinical effects were compared between the two groups.Results The scores on excrement characteristics,defecation difficulty,and time and frequency of defecation were better in the experimental group than those in the control group (t=5.670,4.740,4.170,5.530;P=0.016,0.034,0.039,0.022,respectively).The overall effectiveness rate was 94.9 % (37/39) in the experimental group,which was higher than that (71.8 %,2 8/3 9) in the control group (x2 =4.493,P=0.030).Meanwhile,the levels of 3 SC and 10 SC in the two groups were higher than those before treatment,but the level of 10SR was lower than that before treatment (all P<0.05).Moreover,improvement in 3SC,10SC and 10SR were more significant in the experiment group than in the control group (all P< 0.05).Conclusions Biofeedback training therapy is safe and effective for elderly patients with constipation to relieve constipation symptoms,enhance control of defecation and promote defecation.

ABSTRACT

Objective: To observe the expression of long non-coding RNA myocardial infarction associated transcript (LncRNA-MIAT) in tumor necrosis factor-α (TNF-α) induced endothelial cells (ECs) inflammation in vitroand to study the impact of LncRNA-MIAT on inflammatory regulation. Methods: LncRNA-MIAT expression in ECs was induced by TNF-α at different time and concentration. Expressions of intercellular adhesion molecule-1 (ICAM-1) and LncRNA-MIAT in inflammatory ECs were examined by quantitative real time polymerase chain reaction (qRT-PCR) and Western blot analysis. Moreover, ECs was transfected by siRNAMIAT to observe the effect of LncRNA-MIAT knock-down on ICAM-1 expression. Results: LncRNA-MIAT expression showed the increasing trend by elevated time and concentration of TNF-stimulation. Compared with TNF-α stimulation at 0h, 6h and 12h, LncRNA-MIAT expressions were increased at 24h and 48h of TNF-αstimulation respectively, allP<0.05; compared with TNF-α concentration at 0ng/ml and 0.125ng/ml, LncRNA-MIAT expressions were elevated by TNF-α stimulation at 1.000ng/ml and 10.000ng/ml respectively, allP<0.05. With siRNAMIAT knock-down, TNF-α induced ICAM-1 protein expression was significantly reduced in ECs,P<0.05. Conclusion: LncRNA-MIAT might be involved in ECs inflammatory response and it may play a role to promote inflammation.

ABSTRACT

Objective@#To investigate the feasibility and indication of free grafting of split big toe nail bed for defects of fingernail bed.@*Methods@#From September 2008 to September 2011, 13 cases (4 females and 9 males) with fingernail bed defects(aged 17-38, average, 32) were treated with split big toe nail bed. The split big toe nail was harvested according to the defects size without bone exposure at donor site.@*Results@#Three grafted nail beds were necrotic completely and reconstructed with flaps. The 2 grafted nail bed was partial loss and healed after dressing. All the other grafted nail bed survived completely with primarily healing both in donor sites and recipient sites. The patients were followed up for 6-30 months(average, 14 months). The therapeutic effect was graded as excellent in 8 cases, good in 2 cases and poor in 14 cases(good rate, 76.9%). The nail matrix was excellent in 8 cases, good in 1 case and poor in 1 case(good rate, 90%). The toe nails in donor sites grew well.@*Conclusions@#The single fingernail defect with intact nail matrix can be reconstructed by split toe nail bed graft with good cosmetic and functional effect. There is no malfunction at donor site. The indication should be selected.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the efficacy and the mechanism of Bisacodyl in treating slow transit constipation model rats.</p><p><b>METHODS</b>A total of 30 healthy rats were enrolled. Twenty rats received intragastric diphenoxylate to develop slow transit constipation (STC) model, and 10 untreated rats were set as blank control. STC rats were subdivided into two groups: STC Bisacodyl group (fed with Bisacodyl) and STC control group (common feed). Body weight, number and dry weight of faeces, and intestinal transit time were compared among 3 groups. Interstitial cells of Cajal(ICC) and c-Kit protein expression were measured by immunohistochemical staining. Restults Compared to blank control rats, at 100-day of receiving intragastric diphenoxylate, above 20 rats presented the decrease of body weight and feces number, the increase of dry weight of faeces, and the delay of intestinal transit time, indicating the successful establishment of STC rat model. One month after feeding, compared to STC control group, STC Bisacodyl grap had an increased feces number[(36.6±6.8) pill/day vs. (26.8±6.0) pill/day], decreased dry weight of feces [(150.6±10.5) mg/pill vs. (171.6±16.3) mg/pill] and shortened intestinal transit time [(416.9±50.6) minutes vs. (495.3±66.8) minutes], and the differences were statistically significant(all P<0.05). Dissolution of ICC basement membrane, damage of connection between ICC and surrounding cells, and atrophy of ICC nucleus structure were found in STC control rats. ICC (8.20±1.92 per field] and c-Kit expression (12.68%±2.59% ) in STC control rats were significantly lower than those in blank control rats(36.00±6.25 per field and 71.50 %±8.27%) (P=0.000). Compared to STC control group, the connection between ICC and surrounding cells enhanced obviously, ICC (18.80±3.70 per field) and c-Kit expression (45.91%±6.80%) were significantly higher in STC Bisacodyl group (all P=0.000).</p><p><b>CONCLUSION</b>Bisacodyl treatment can relieve STC symptoms, which may be associated with increased ICC number and c-Kit protein expression.</p>

ABSTRACT

Objective To survey the quality of life and mental health status of pre-post household service workers who held a high school graduation certificate or above and had been engaged in different work.Methods A total of 254 household service workers completed Symptom Checklist 90 (SCL-90) and World Health Organization Quality of Life-Brief (WHOQL-BREF).Statistical analyses of variance and correlation were carried out to evaluate mental health status and quality of life of the participants.Results Abnormal score of SCL-90 was found in 8% of household service workers who had graduated from high school.The psychological score of household service workers who had worked as workers(60.59± 2.18)was lower than those who had worked as service workers (66.23 ± 12.13) or peasants (65.40 ± 12.24) (P <0.05).Almost all factors of SCL-90 were negatively correlated with WHOQL-BREF (correlation coefficient -0.140 to-0.376).Conclusions Our data suggest that mental health status can be well in pre-post household service workers who held a high school graduation certificate or above.Special training should be given to the household service workers in accordance with their working experience.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the androgenic and anti-androgenic effects of GH (growth hormone) transgenic carp in male rats.</p><p><b>METHODS</b>Hershberger assay was carried out in castrated male SD rats aged 4-5 weeks. Testosterone propionate (TP) (0.4 mg/kg BW) was administrated for a positive control, GH transgenic carp (3.0 g/kg BW)+TP (0.4 mg/kg BW), parental carp (3.0 g/kg BW) + TP (0.4 mg/kg BW), and flutamide (Flu) (3.0 g/kg BW) were used for negative controls, and vehicle was administered orally for a blank control. All groups were administrated for 10 consecutive days. At the end of the test, animals were anesthetized, then weights of accessory sex organ were measured. Serum testosterone (T), luteinizing hormone (LH), and Follicle-Stimulating Hormone (FSH) levels were detected.</p><p><b>RESULTS</b>The weights ratios of the accessory sex organs and body weights showed no significant differences between the solvent control and the GH transgenic carp-treated groups. Serum concentrations of FSH, LH, and T of the rats treated with GH transgenic carp + TP showed no significant changes, compared with those treated with TP only.</p><p><b>CONCLUSION</b>GH transgenic carp does not have any androgenic agonist or antagonist properties in vivo screening tests.</p>

ABSTRACT

BACKGROUND: In many parts of the body, muscle diastolic and contractive activities can promote the body's energy metabolism. Furthermore, the muscles also play an irreplaceable role in maintaining body posture and joint stability. OBJECTIVE: To summarize the rules of muscle function and aging changes, to search for research related to the trainability of old muscle function, and to provide references for muscular anti-aging research in field of sports medicine. METHODS: Taking senescence, aging, muscle function, exercise, training in English as the search terms, Pubmed database from January 2000 to May 2009 was retrieved; Taking aging, muscle function, training in Chinese as the search terms, CNKI database from January 2000 to May 2009 was searched. Literatures were limited to English and Chinese languages. Inclusive criteria: research closely related to the trainability of motor function and muscle metabolic function in the elderly; Exclusive criteria: obsolete and lack of credibility of the literature. RESULTS AND CONCLUSION: Totally 67 literatures were screened out by computer, according to inclusion and exclusion criteria, 31 documents of which were involved in the analysis. Based on the recent research On muscle function, the muscle function can be summarized into three aspects: motor function, metabolic function and maintenance function. The elderly muscle aging is mainly showed in these 3 aspects. How to reconstruct these functions through resistance training? The concerning data were summarized in this paper, to verify the trainability of ageing's muscle motor and metabolism functions, but no study indicates that the maintenance function can be reconstructed, and deserves further studies of sports science.

ABSTRACT

BACKGROUND: As a plant in valerianaceae, patrina villosa juss, which characterizes by acrid and bitter in taste and cold in nature, has been proved that its extract has effect on central inhibition.OBJECTIVE: To observe the effect of patrina villosa juss extract on hypoxia tolerance of mice and acknowledge whether it has dosage-dependence or not.DESIGN: Randomized controlled animal study.SETTING: Pharmacological Department and Pathological Department of Gannan Medical College.MATERIALS: The experiment was completed at the Laboratory of Scientific Research Center of Gannan Medical College from March to April 2005. A total of 100 healthy adult Kunming mice were selected in three hypoxia experiments.METHODS: ① Hypoxia tolerance experiment under normal pressure:Forty mice were randomly divided into 4 groups. Mice were injected intravenously with 2 μL/g saline in saline group, with 0.02 mg/g propranolol solution (10 g/L) in propranolol group, with 0.02 mg/g patrina villosa juss extract in 0.02 mg/g patrina villosa juss group, and 0.04 mg/g patrina villosa juss extract in 0.04 mg/g patrina villosa juss group, respectively. Twenty-five minutes later, mice were put into wide mouthed bottle with the volume of 250 mL and the bottle was enclosed to observe the survival time. ② Rapid decapitation experiment: Thirty mice were randomly divided into 3 groups. Mice were injected intravenously with 2 μL/g saline in saline group, with 0.02 mg/g patrina villosa juss extract in 0.02 mg/g patrina villosa juss group, and 0.04 mg/g patrina villosa juss extract in 0.04 mg/g patrina villosa juss group, respectively. Twenty-five minutes later, heads of mice were cut rapidly to record the time from decapitation to the last gasp. ③ Experiment for ligating bilateral common carotid artery: Thirty mice were randomly divided into 3 groups. Mice were perfused with 2 μL/g saline in saline group, with 0.01 mg/g patrina villosa juss extract in 0.01 mg/g patrina villosa juss group, and 0.015 mg/g patrina villosa juss extract in 0.015 mg/g patrina villosa juss group, respectively, once a day for 7 days in total. Seven days later, bilateral common carotid artery was ligated to observe time of respiratory arrest.MAIN OUTCOME MEASURES: ① Survival time of hypoxia tolerance;② time from decapitation to the last gasp; ③ time from ligating bilateral common carotid artery to respiratory arrest.RESULTS: A total of 100 mice were involved in the final analysis. ① Survival time of hypoxia tolerance under normal pressure: Time in 0.02 mg/g and 0.04 mg/g patrina villosa juss groups was longer than that in saline group [(57.8±4.6), (76.2±4.9), (42.5±3.6) minutes, P < 0.05, 0.01], but there was no significant difference from that in propranolol group (P > 0.05).The higher the dosage was, the longer the survival time was. ② Gasping time of decapitation mice: Time in 0.02 mg/g and 0.04 mg/g patrina villosa juss groups was longer than that in saline group [(22.1 ±1.6),(25.3±2.2), (18.6±0.8) s, P < 0.05, 0.01], and the higher the dosage was, the longer the survival time was. ③ Time of respiratory arrest: Time in 0.01 mg/g and 0.015 mg/g patrina villosa juss groups was longer than that in saline group [(123.4±25.1),(142.2±30.2), (86.0±12.8) s, P < 0.05, 0.01], and the higher the dosage was, the longer the survival time was.CONCLUSION: Patrina villosa juss extract can improve symptom of myocardial hypoxia induced by cerebral hypoxia, whole-body hypoxia and increase of myocardial oxygen consumption; moreover, the higher the dosage is, the more remarkable the effect is. The mechanism is of possibility that patrina villosa juss extract can improve myocardial and cerebral oxygen consumption.

ABSTRACT

<p><b>OBJECTIVE</b>The relationship between polymorphisms of ALAD and VDR genes and individual susceptibility of lead poisoning was investigated in children highly-exposed to lead.</p><p><b>METHOD</b>Four hundred and sixty-nine children were recruited into this study and the blood lead, ZPP, hemoglobin as well as three physical developmental indexes (head circumference, height and weight) were measured. VDR and ALAD gene polymorphisms were analyzed by the methods of PCR-RFLP.</p><p><b>RESULTS</b>The subjects with ALAD2 allele had higher ZPP level (10.12 micro mol/L vs 12.87 micro mol/L) (P = 0.017). The subjects with B allele has larger head circumference than only with b allele (51.19 cm vs 50.75 cm) (P = 0.028).</p><p><b>CONCLUSIONS</b>It was suggested that the ALAD gene polymorphism modified the relationship between blood lead and ZPP and the VDR gene variants influenced the skull development in children living under lead-polluted environment. The polymorphism of ALAD and VDR genes might serve as the molecular inherited factors modifying the susceptibility of lead poisoning.</p>

ABSTRACT

AIM: To establish a Reversed-Phase high performance liquid chromatography method for determination of plasma ropivacaine in dog.METHODS:The sample was deproteinated by acetonitrile and determined with RP-HPLC and ultraviolet detection.the analytical column was Intertsil C18 column((4.6) mm?250 mm,5 ?m).The mobile phase consisted of methanol:H_2O(5050),the detection wavelength was 215 nm and the flow rate was(1.4)(ml?min~(-1)),the column temperature was 20 ℃,the sensitivity was 0.02AUFS respectively.The volume of sample for detection is 10 ?l.RESULTS:The linear range was(0.1)-25(?g?ml~(-1))(r=(0.9992)). The recovery of ropivacaine was(91.2)%-(93.6)%,RSD was(2.10)%-(3.40)%(n=6).The detection limit was(0.05)(?g?ml~(-1)).the intra-day and interday precisions of assay for ropivacaine was(1.35)%-(2.88)% and(1.80)%-(3.76)%.The quantative analysis of ropivacaine in plasma was not interfered by impurities.CONCLUSION: The assay method was shown to be suitable for determination the concentration of ropivacaine in plasma.The method is simple,accurate,sensitive and reproducible.

ABSTRACT

OBJECTIVE:To set up a HPLC method for the determination of the contents of notoginsenoside R1,ginsenoside Rg1 and Rb1 in Sanqishangyao tablets.METHODS:It was performed on VP-ODS (150mm?4.6mm,5?m),and the mobile phase was acetonitrile-water with gradient elution system(0~20min(20∶80),20~26min(40∶60),26~35min(20∶80)).The flow rate was 1.0mL?min-1 and the column temperature was room temperature.The detection wavelength was 203nm.RESULTS:The liner range of notoginsenoside R1 was 0.261~7.83?g(r=0.999 9)and the average recovery was (101.12?2.03)%(RSD=2.1%).The liner range of ginsenoside Rg1 was 0.602 5~18.075?g(r=0.999 7)and the average recovery rate was (97.22?3.33)%(RSD=1.9%).The liner range of ginsenoside Rb1 was 0.537 5~16.125?g(r=0.999 6)and the average recovery rate was(100.71?2.4)%(RSD=1.6%).CONCLUSION:The method is suitable for the content determination of notoginsenoside R1,ginsenoside Rg1 and Rb1 in Sanqishangyao Tablets.
